1. Copy of the front page only of form 8130-6 SIGNED and DATED!!
2. Copy of your signed and Notarized Form 8130-12 Statement of Eligibility.
3. Copy of your program letter.
4. Copy of your Weight and Balance for airplanes or Weight and Loading for Trikes and PPCs.
5. Copy of your Registration certificate or Temporary Registration.... NOT THE PINK COPY  or N-NUMBER RESERVATION.
DO NOT fax photos of your plane or inspections checklists!
I will need three original photos: front, side and rear views when I do your inspection.  They must be photos of your aircraft ready to inspect with N-numbers applied not drawings out of the manual.

Once your aircraft is complete and ready to fly call me to schedule an inspection.  To help you in a preparing the inspection I have included my Amateur Built Inspection Checklist  and the new Kit Evaluation checklist.     Those checklists are the same ones I will be using so it would be a good idea to download them and you and a friend go through the aircraft in a critical way to see if there are any items that may not be satisfactory.    The Kit Aircraft checklist is only needed if the aircraft was constructed from a kit and not built from plans.  Some items may not be applicable so if you don't have, for instance a retractable gear, ignore that item.  I will be checking your builders log and other documents of your building project.  Also all bolts must have at minimum 1 thread showing.  If I find bolts that don't have 1 thread showing they will have to be replaced prior to issuing the Airworthiness certificate.
There will be an operational check of the engine so be prepared to install the cowling and run the engine to check the items on the checklist under "Operational Checks".

I will also need to have three glossy photos of your aircraft.  A front, side and rear view.  These should  be 4"x 6" size.
